Telehealth services are available only to patients who are physically located in Ohio at the time of the visit. In-person services are offered in Kentucky. Patient location is verified during scheduling/intake and again at the start of the visit.

3. Scheduling and Communications

When you request an appointment, you agree to provide accurate and complete information. You may receive communications related to scheduling, forms, and care coordination by phone, email, and/or secure messaging.

4. Payments

If payment is collected for services, deposits, or packages, you agree to pay the amounts presented at checkout or as otherwise agreed in writing. Payment terms may vary by service.

5. Cancellations and No-Shows

Appointment cancellation and rescheduling policies may be provided at the time of booking or in your intake paperwork. If a fee applies, it will be disclosed prior to charging.
